After that, you can download the Teaching Excellence Awards Form (WORD).

Please note that this year’s nomination form and guidelines have been revised. The revised nomination steps can be found in the full award guidelines. The deadline for receipt of the complete nomination package is now February 15, and we are requesting that nominations be received electronically only as a single .pdf rather than as multiple files.
